Horizon Scholar Program Application 2022

Congratulations on your admission to Lake Forest College and nomination to apply for the 2022 Horizon Scholar Program. Please complete the application below, noting that applicants must submit the following:

2 writing samples (prompts provided below).
Supplemental recommendation letter addressing the applicant's interest in and suitability for a career in the Health Professions.

Your application progress cannot be saved, so it is recommended that you do not start the application until you have prepared the required supplemental materials. The submission deadline is January 15, 2022.

Successful candidates as determined by the selection committee will be invited for a final round interview with Dr. Shubhik DebBurman, Senior Director for Science Partnerships, Professor of Biological Sciences, and Chair of Neuroscience and Mike Cohen, Director of Admissions and Strategic Development.

Two students selected as program recipients will receive annual funding to fully cover all tuition, fees, and book expenses. Horizon Therapeutics will assign a mentor to each scholar, provide professional development and networking opportunities, and arrange periodic engagement with other Horizon Scholars/Fellows at Rosalind Franklin University, Howard University, and the Asian University for Women.
